The following is a compilation of sources which may serve as additional examples to help you prepare your office.  Below is a short explanation of each resource.

 

CDPHE PanReady GUIDELINES FOR MEDICAL OFFICE PANDEMIC READINESS:  The purpose of this plan is to prepare your practice for the possibility of pandemic influenza or other disaster. This plan was developed under the guidelines of CDPHE, CDC and OSHA. It is used as a model that your practice may follow to establish its own Emergency Operations Plan.

 

Clinic_Template_EOP:  This is the Word based template used to establish a personalized Emergency Operations Plan for your practice.  This template is generalized, but can be specific enough, to fit any situation.

 

FEMA Form 90-49: Request for Public Assistance Form.  This is a PDF of the form you would initially fill out to request FEMA assistance.

 

KMA Disaster Plan:  As an example of a very simplified plan for a practice to follow.  The Kentucky Medical Society developed this basic plan for practices to follow but without much detail.

 

No. 323, Public Assistance Handbook, FEMA:  The Handbook describes how practices apply for assistance under the Public Assistance Program.  The Applicant Handbook was developed to provide easy to follow instructions on how to apply for Public Assistance grants.

 

PanFlu Booklet:  Distributed last year, it has proven to be a good source on how to prepare your home and clinic specifically for an epidemic.  The booklet could be applied to other emergencies.  It provides very easy to follow checklists.

 

ToolKit HVA:  Kaiser Permanente created a Hazard Vulnerability Assessment (HVA) Excel File.  To perform a HVA on your practice to identify what are the main threats or hazards to your clinic and to develop your Emergency Operations Plans Annexes, please email tammy@epcms.org.
